Hollow, Tired Eyes on GLP-1: Why It Happens and What Helps

Hollow eyes during GLP-1 weight loss are mostly about lost volume, made more visible by very thin under-eye skin. How to tell hollows from dark circles and puffiness, what eye-area skincare can do, and when to see someone.

TL;DR: THE ESSENTIAL POINTS

  • Hollow eyes during weight loss are mostly about volume, not skin. Fat loss in the face can deepen the tear trough and create shadow under the eyes.1
  • The skin under the eyes is among the thinnest on the body. That is why changes underneath show through so quickly.2
  • Skincare cannot refill lost volume. What it can do is hydrate thin, dry skin so it looks smoother and less crepey. Hyaluronic acid creams have been shown to do that around the eyes.5
  • Not all "tired eyes" are the same problem. Hollows, dark pigment, puffiness and lack of sleep each look different and need different help.2,4
  • Sudden changes in eyesight are not a skin issue. If you take semaglutide and notice sudden vision loss, the MHRA advises going to eye casualty or A&E urgently.8

"You look tired." Even when you are not.

It is one of the comments people on GLP-1 medicines mention most, and it stings because it often arrives just as they are feeling better than they have in years. The weight is coming off, energy is up, and the face in the mirror looks more drawn around the eyes than it did before.

The under-eye area is where facial change tends to show first. Understanding why makes it much easier to work out what is worth doing, and what is a waste of money.

The key question.

Is the problem a hollow (something missing underneath), or the skin (thin, dry, crepey or dark)? Most people have a bit of both. They need different things.

This article covers why eyes can look hollow during weight loss, how to tell volume loss from skin changes, what the evidence says about eye-area skincare, and when to see a professional instead.

01. WHY EYES CAN LOOK HOLLOW DURING WEIGHT LOSS

THE FACE LOSES FAT TOO

A 2025 review of eye-area changes linked to GLP-1 medicines describes hollowness around the eyes, along with fat loss in the temples and cheeks, as effects of rapid weight loss rather than a confirmed direct effect of the drug.1 Another 2025 review describes the facial volume loss that happens when rapid fat reduction depletes the fat under the skin.3

When the fat in the cheek just below the eye shrinks, the groove between the lower lid and the cheek, the tear trough, becomes more visible. The 2025 eye review links loss of this superficial midface fat to more pronounced tear troughs, citing one small imaging study that measured an 11% loss of superficial midface volume in people on GLP-1 medicines.1

WHAT IS NOT KNOWN YET

"Sunken eyes" are often mentioned as part of so-called Ozempic face, but the same review notes that changes to the fat inside the eye socket itself had not been studied directly, and that most of the evidence so far is small studies, case reports and expert opinion.1 The term "Ozempic face" is not a medical diagnosis.3

WHY THE UNDER-EYE AREA SHOWS IT FIRST

A review of under-eye dark circles describes the lower eyelid skin as one of the thinnest parts of the body, with very little tissue beneath it, so structures underneath show through and ageing changes are more pronounced than elsewhere on the face.2 When volume is lost, hollows cast shadows, especially under overhead lighting.2

A lot of what reads as "tired" is shadow. Light falls into a hollow and the area looks darker, even if the skin itself has not changed colour.

02. HOLLOW, DARK, PUFFY OR TIRED? TELLING THEM APART

The same review lists several separate causes of under-eye darkness: volume loss and hollows, pigment in the skin, visible blood vessels, and fluid that collects in the lower lid, often worse in the morning or after a salty meal.2 Lack of sleep adds its own signs. In one study, faces photographed after 31 hours without sleep were rated as having more hanging eyelids, puffier eyes, darker circles and paler skin.4

What you see Likely cause What can help
A groove or dip that looks darker under overhead light Volume loss and shadow1,2 Skincare will not refill it. Fillers or fat transfer are the clinical options2
Fine, crepey lines and dryness Thin, dehydrated skin2 Hydrating skincare, which has the best evidence of anything topical here5
Brown or grey colour that stays in any light Pigment in the skin2 Daily sun protection; a GP or dermatologist if it is changing
Puffiness, worse in the morning Fluid in the lower lid2 Sleep, less salt in the evening, a cool compress
Everything worse after bad nights Sleep loss4 Sleep. No product competes with it

Try this: stand under a ceiling light, then face a window. If the darkness fades a lot when the light is in front of you, shadow from a hollow is a big part of it.

03. WHAT EYE-AREA SKINCARE CAN ACTUALLY DO

I am going to be blunt, because this category is full of overpromising. No eye cream, serum or patch can put back fat that has gone. What skincare can do is improve the skin over the top of the hollow: how hydrated, smooth and comfortable it is. On skin this thin, that makes a visible difference.

HYALURONIC ACID AROUND THE EYES

In a randomised trial, 76 women applied a 0.1% hyaluronic acid cream around one eye and a plain cream around the other, twice a day for 60 days. Every hyaluronic acid version improved skin hydration and elasticity compared with the plain cream, and the lower molecular weight versions also reduced wrinkle depth.5

NIACINAMIDE AROUND THE EYES

Niacinamide is vitamin B3. In a split-face study of 30 Japanese women with wrinkles around the eyes, a cosmetic containing 4% niacinamide was applied to one side and a version without it to the other for eight weeks. Dermatologists rated 64% as markedly or moderately improved on the niacinamide side, and skin roughness fell. One person stopped because of mild irritation.6

That is a small study on a different product. It explains why niacinamide is a sensible ingredient for the eye area. It does not mean any other product, including ours, will produce the same result.

WHAT ABOUT CAFFEINE AND PEPTIDES?

The dark circles review cites a placebo-controlled trial in which a caffeine gel reduced lower-lid puffiness and pigmentation. For peptides, it gives a rationale but no clinical data on dark circles.2

Be gentle with this skin.

Pat, do not rub. Use your ring finger. Keep strong actives away from the lash line, and stop anything that stings.

04. WHEN TO SEE SOMEONE INSTEAD

IF YOUR EYESIGHT CHANGES

This is not about appearance, but it matters. The MHRA has said that semaglutide (Wegovy, Ozempic and Rybelsus) is linked very rarely, in up to 1 in 10,000 people, to a serious eye condition called NAION that can affect sight. If you notice sudden loss of vision or eyesight that gets worse very quickly, go to eye casualty or A&E urgently.8

IF THE HOLLOW IS WHAT BOTHERS YOU

Hyaluronic acid filler is described in the dark circles review as a treatment of choice for under-eye volume loss, partly because it can be dissolved.2 It is not risk-free. An oculoplastic surgeon at Moorfields Private Eye Hospital lists bluish discolouration, persistent swelling, lumps, infection, complications with later eyelid surgery and, rarely, irreversible loss of vision if filler enters a blood vessel.7

If you are still losing weight, your face is still changing. Many people choose to wait until their weight is stable before considering anything injected. If you do go ahead, choose a medically qualified practitioner and ask about complications before you book.

05. AN EYE-AREA ROUTINE FROM THE SKIN IN EVOLUTION™ SYSTEM

These are the steps that matter most for the under-eye area. They work on the skin, not the volume underneath.

Renaissance Infusion Eye Patches box with gold hydrogel patches
Routine extension: Targeted eye care

RENAISSANCE INFUSION EYE PATCHES

Hydrogel under-eye patches with niacinamide, sodium hyaluronate (a form of hyaluronic acid), glycerin, panthenol and aloe. Fragrance free. Apply one pair to clean, dry skin under the eyes, leave for 15 minutes, then remove and press the remaining essence in. Seven pairs per box; many people use them a few times a week.

Why it suits tired-looking eyes

They work on the skin over the hollow, not the volume underneath. Hyaluronic acid and niacinamide were chosen because both have been studied around the eyes, in other products, for hydration and the look of fine lines.5,6 That research explains the choice. It does not mean the patches will give the same results.

Hydration Continuum Gel bottle and box with drops of gel
Stage 1: Morning hydration

HYDRATION CONTINUUM GEL

Hyaluronic acid with sodium PCA and aloe, straight after your morning shower. Use a little around the orbital bone, not on the lid itself.

Why it suits the under-eye area

Under-eye skin is among the thinnest on the body, so dryness shows there first.2 A small amount of a light gel, patted along the orbital bone, is a gentle first layer of hydration with nothing heavy near the eyes.

Peptide Solstice Day Cream tube
Routine extension: Daytime

PEPTIDE SOLSTICE DAY CREAM

A lightweight peptide moisturiser for the face and neck with hexapeptide-11, dipalmitoyl hydroxyproline, hyaluronic acid and jojoba. Follow with sunscreen.

Why it suits the under-eye area

Pigment under the eyes can darken with sun exposure,2 so the morning routine matters. This cream sits comfortably under sunscreen, which is the step that protects this area most.

ElastiK° Skin LED Face + Neck Mask glowing red on a display head
Stage 3: LED (optional)

ELASTIK° SKIN LED FACE + NECK MASK

234 LEDs across the face and neck, used on clean, dry skin with the silicone eye protectors in place and your eyes closed. Start at 5 minutes and build to 10. Not suitable during pregnancy. UK delivery only.

Why it suits tired-looking eyes

In a randomised trial using a different LED device, red light reduced wrinkle volume around the eyes but did not change hydration or elasticity.9 No study has been done on our mask, and no light can refill a hollow, so treat it as an optional extra.

Resilience Matrix Night Cream pump bottle and box with a swatch of cream
Stage 5: Bedtime

RESILIENCE MATRIX NIGHT CREAM

Shea and cocoa butter with plant glycosphingolipids, as you get into bed, on the face, neck and décolletage.

Why it suits the under-eye area

There is very little tissue under the lower lid, so the area can feel tight by morning.2 A small amount pressed along the cheekbone, not on the lid, keeps thin skin comfortable overnight.

In our pre-launch trial of the topical routine, all 11 participants said their skin felt more hydrated after six weeks, and 9 of 11 said it felt more comfortable (see the evidence).

06. FAQ

WHY DO MY EYES LOOK SUNKEN ON OZEMPIC OR MOUNJARO?

Mostly because of fat loss in the face, which deepens the hollow under the eye and creates shadow.1 The thin skin there makes it more visible.2

WILL MY EYES STOP LOOKING HOLLOW WHEN MY WEIGHT STABILISES?

There is no good research on that yet.1 The face usually stops changing once weight is stable, which is the best time to judge what you are left with.

CAN EYE CREAM FIX UNDER-EYE HOLLOWS?

No. It cannot replace lost volume. It can make the skin over the hollow better hydrated and smoother, which helps it look less crepey.5

DOES NIACINAMIDE WORK UNDER THE EYES?

One small study found a 4% niacinamide cosmetic improved the look of wrinkles around the eyes over eight weeks.6 It is promising, not proven for everyone.

ARE UNDER-EYE FILLERS SAFE?

They are widely used, but they carry risks including swelling, lumps, discolouration and, rarely, loss of vision.7 See a medically qualified practitioner.

MY EYESIGHT HAS CHANGED SINCE STARTING SEMAGLUTIDE. WHAT SHOULD I DO?

If it is sudden or getting worse quickly, go to eye casualty or A&E urgently.8 For anything else, speak to your prescriber or optometrist.

WHAT TO DO NOW

Work out which problem you have, then match the help to it.

  1. Do the light test: overhead light, then window light. Shadow that fades is a hollow.
  2. Protect your sleep: nothing in a bottle beats it for tired-looking eyes.
  3. Hydrate the skin daily: gentle, patted in, never rubbed.
  4. Wear sunscreen: pigment under the eyes can darken with sun.
  5. Take vision changes seriously: sudden changes mean eye casualty or A&E, not a new eye cream.
  6. Add targeted eye care: see the Renaissance Infusion Eye Patches or take the skin quiz.
