Ozempic Neck: Why Neck Skin Loosens First and How to Look After It

"Ozempic neck" is a media label, but looser, crepey neck skin during weight loss is a real worry. Why the neck often shows change first, what skincare can and cannot do, and where surgery fits.

TL;DR: THE ESSENTIAL POINTS

  • "Ozempic neck" is a media label, not a diagnosis. Even the better-known term "Ozempic face" lacks medical validation, according to a 2025 dermatology review.1
  • Fat loss is part of it, not all of it. Rapid fat reduction is thought to play a big role in facial changes on GLP-1 medicines, but researchers say other processes, including collagen changes, are likely involved.1,2
  • The neck is the most forgotten part of most routines. The NHS says sunscreen should go on the face, neck and ears, and daily sunscreen has been shown to slow visible skin ageing.3,4
  • Skincare cannot tighten loose skin. It can improve how neck skin looks and feels. Removing excess skin needs surgery.5
  • Start early and be consistent. Take your face routine down to your neck and chest every day.

A lot of people in our community tell us the same thing: their face looked fine, then one day a photo from the side showed their neck.

Loose skin under the chin. Lines that were not there before. A crepey texture that make-up cannot hide and polo necks suddenly feel like the answer to.

The internet has a name for it, "Ozempic neck" or "turkey neck". It is not a medical term, but the worry behind it is real, and it is one of the most common questions we get from people losing weight on Mounjaro, Wegovy or Ozempic.

It is not only a GLP-1 issue.

Neck skin also changes with age, sun and menopause. If you are not on a weight-loss medicine, most of this article still applies to you.

This article covers what is happening, why the neck often shows change before the face, what helps, what does not, and where surgery fits.

01. WHAT PEOPLE MEAN BY "OZEMPIC NECK"

When people talk about Ozempic neck, they usually mean some mix of:

  • looser skin under the chin and along the jaw
  • more visible lines or bands on the front of the neck
  • a finer, crepey texture on the neck and chest

The term comes from "Ozempic face". A 2025 review in the journal Diseases describes Ozempic face as facial volume loss that happens when rapid fat reduction depletes the fat under the skin, with deeper wrinkles and sagging. The same review is clear that the expression lacks medical validation and that the precise mechanism remains unclear.1 The review does not discuss the neck. We have not found any study looking at neck skin specifically in people taking GLP-1 medicines.

There is no research on "Ozempic neck". What we have is research on the face, on skin and fat after weight loss, and on what sun and age do to skin. That is enough to explain most of it.

02. WHY THE NECK CAN CHANGE FIRST

THE FAT THAT WAS HOLDING IT OUT HAS GONE

Quick weight loss on semaglutide is thought to play a crucial role in the facial changes people notice, as the fat under the skin is depleted.1 There is fat under the chin and along the jaw too. It is reasonable to expect the same thing there: when that fat shrinks quickly, the skin over it can be left looser, and the neck is where that shows most from the side.

IT IS NOT ONLY FAT

A 2025 review in Endocrine argues that facial ageing on GLP-1 medicines is not solely due to lost fat. It discusses possible effects on cells in the skin and the fat beneath it, including reduced support for collagen, and concludes that the mechanisms still need to be worked out.2 The Diseases review also mentions collagen breakdown and changes in elasticity as proposed contributors.1

THE NECK IS THE FORGOTTEN ZONE

Many people stop their skincare and their sunscreen at the jawline. The NHS says sunscreen should be applied to all exposed skin, including the face, neck and ears.3 In a randomised trial of 903 adults under 55, those who used sunscreen every day showed no detectable increase in skin ageing over four and a half years, compared with those who used it when they chose to.4 Years of the neck missing out add up.

AGE AND HORMONES

Many people losing weight are also in their forties and fifties. For women, menopause brings its own collagen loss, estimated at around 30% in the first five years.6,7 If you are losing weight through perimenopause or menopause, the neck is dealing with both at once. We explain the menopause side in Menopause Skin Changes: What Happens to Collagen, and What Helps.

Factor Can you change it? What helps
Fat loss under the chin Partly. The pace of weight loss is a conversation for your prescriber Raise it at your medication review if it is worrying you
Collagen and elasticity Not directly Daily skincare supports how the skin looks and feels
Sun exposure Yes Sunscreen on the neck and chest, not just the face3,4
Age and menopause No, though menopause care is worth discussing with your GP A consistent routine and sun protection6,7
Excess skin Only with surgery A consultation with a registered plastic surgeon5,8

03. WHAT ACTUALLY HELPS, AND WHAT DOES NOT

I am going to be blunt, because this category is full of overpromising.

WHAT SKINCARE CAN DO

Good skincare can improve hydration, texture and how smooth neck skin looks. It can make crepey skin catch the light differently, and it can keep skin comfortable. That is worth having.

WHAT SKINCARE CANNOT DO

No cream, serum or device removes loose skin. If you see a neck cream promising to tighten or lift a loose neck, it is promising something skincare cannot do.

WHERE SURGERY FITS

The NHS describes a facelift as surgery designed to reduce loose skin around the lower half of the face, mainly the jowls, and the neck. It quotes costs from a few thousand pounds for a mini facelift to around £10,000 for a face and neck lift, plus consultations and follow-up. Recovery takes around two to four weeks, the full result can take six to nine months to show, and risks include nerve injury, asymmetry and visible scarring.5

If you are thinking about it, the NHS advises checking that your surgeon is on the General Medical Council register, that a clinic in England is registered with the Care Quality Commission, and having a consultation with the person who will operate before you pay anything.8

A word on gadgets.

Neck massagers, tightening tapes and at-home "lifting" devices are everywhere on social media. A UK dermatologist's review of menopausal skin lists LED devices among products that lack high-quality evidence.6 We sell an LED mask that covers the neck, and I would still rather you heard that from us.

HOW TO LOOK AFTER YOUR NECK DAY TO DAY

  • Take every step down to the collarbone. Anything that goes on your face goes on your neck and chest too.
  • Apply straight after the shower. The NHS says moisturisers are best applied after washing, as soon as skin is patted dry, and smoothed on rather than rubbed.9
  • Be gentle. Use light strokes. There is no need to drag or pull the skin.
  • Sunscreen every day you are outdoors. SPF 30 or higher with at least 4-star UVA, on the neck as well as the face.3
  • Take monthly photos from the side. Same light, same angle. It is the only way to judge fairly.

04. A NECK-FOCUSED SKIN IN EVOLUTION™ ROUTINE

The Skin in Evolution™ System was made for skin going through rapid change, including GLP-1 and other weight loss, perimenopause and menopause, and after pregnancy. Every step below is designed to be used on the neck as well as the face.

Hydration Continuum Gel bottle and box with drops of gel
Stage 1: Morning hydration

HYDRATION CONTINUUM GEL

Two forms of hyaluronic acid with sodium PCA and aloe. Straight after your morning shower on the face, neck, décolletage and arms.

Why it suits neck skin

Neck skin is easy to miss when you moisturise. A light gel that goes on straight after the shower, as soon as skin is patted dry,9 makes it simple to take hydration down to the collarbone every day.

Shop Hydration Continuum Gel →
Collagen Architect Serum pump bottle and box with drops of serum
Stage 2: Morning firmness

COLLAGEN ARCHITECT SERUM

A silky serum with hyaluronic acid and sodium PCA that supports the look of firmer, more hydrated skin. Smooth it up the neck and over the décolletage, not just the face.

Why it suits neck skin

It is made for the face, neck and décolletage, so the area that shows change from the side gets the same morning step as your face. It sits under sunscreen, which the neck needs every day too.3

Shop Collagen Architect Serum →
ElastiK° Skin LED Face + Neck Mask glowing red on a display head
Stage 3: LED (optional)

ELASTIK° SKIN LED FACE + NECK MASK

234 LEDs across the face and neck. Use it on clean, dry skin in the evening before your skincare, or in the morning before anything else. Start at 5 minutes and build to 10. Not suitable during pregnancy. UK delivery only.

Why it suits neck skin

It covers the neck as well as the face, so the neck is not left out of this step. As above, the evidence for LED is limited, so treat it as an optional extra.6

Regenesis Complex pump bottle and box with drops of serum
Stage 4: Evening peptide step

REGENESIS COMPLEX

A peptide serum with hexapeptide-11 and dipalmitoyl hydroxyproline, for the face, neck, décolletage or anywhere skin feels less firm. A small amount covers the whole neck.

Why it suits neck skin

Neck skin that feels less firm is exactly what this step was made for, and because it is for face and body, it can go from the jaw down to the chest in one go.

Shop Regenesis Complex →
Evolve Serum dropper bottle and box with drops of golden oil
Stage 4: Evening renewal

EVOLVE SERUM

Bakuchiol with rosehip, evening primrose and jojoba oils, straight after Regenesis Complex.

Why it suits neck skin

For crepey texture on the neck and chest. Bakuchiol is a gentler alternative to retinoids, in a nourishing oil base. Start on a few evenings a week and build up.

Shop Evolve Serum →
Resilience Matrix Night Cream pump bottle and box with a swatch of cream
Stage 5: Bedtime

RESILIENCE MATRIX NIGHT CREAM

A rich cream with shea and cocoa butter and plant glycosphingolipids, applied as you get into bed. Face, neck and décolletage.

Why it suits neck skin

Richer products suit night-time.9 Taking your night cream down over the neck keeps that skin comfortable and supports the barrier overnight.

Shop Resilience Matrix Night Cream →
Peptide Solstice Day Cream tube
Extra: Daytime moisture

PEPTIDE SOLSTICE DAY CREAM

A lightweight peptide moisturiser with hexapeptide-11, hyaluronic acid and jojoba oil, for the face and neck each morning after your serums.

Why it suits neck skin

It is designed for the face and neck, so the neck gets a daytime moisturiser too, not just whatever is left on your fingers. Follow with SPF 30 or higher.3

In our small pre-launch trial, 11 people used the routine for six weeks. All 11 said their skin felt more hydrated, and 9 of 11 said it felt more comfortable. That is about how skin felt, not about loose skin, and the details are on our evidence page.

05. FAQ

WHAT IS OZEMPIC NECK?

It is an informal name for looser, more lined or crepey neck skin during weight loss on medicines like Ozempic, Wegovy or Mounjaro. It is not a medical diagnosis, and even "Ozempic face" lacks medical validation.1

IS OZEMPIC NECK PERMANENT?

It depends on how much loose skin there is. Texture and hydration can improve with consistent care. Excess skin does not go away with skincare, and removing it needs surgery.5

DOES MOUNJARO CAUSE NECK SAGGING TOO?

Most of the research and reporting has focused on semaglutide. Rapid weight loss is thought to play a big part, though other processes are likely to be involved too.1,2 Any medicine or diet that leads to fast fat loss could have a similar effect.

CAN A NECK CREAM TIGHTEN LOOSE SKIN?

No. Skincare can support how neck skin looks and feels, especially texture and hydration. It cannot tighten or lift loose skin.

HOW MUCH DOES A NECK LIFT COST IN THE UK?

The NHS quotes up to around £10,000 for a face and neck lift, plus consultations and follow-up care.5 Prices vary by surgeon and clinic.

WHEN SHOULD I START LOOKING AFTER MY NECK?

Now. Sunscreen and a daily routine that includes the neck are easier to keep up than to catch up on.3,4

WHAT TO DO NOW

Your neck needs the same attention as your face. Here is the order.

  1. Take your routine down to your collarbone: every product, every day.
  2. Wear sunscreen on your neck: SPF 30 or higher with 4-star UVA.
  3. Take a side-on photo once a month: same light, same angle.
  4. Talk to your prescriber if the pace of change worries you: how fast you lose weight is a medical decision.
  5. Build a routine that covers face and neck: take the skin quiz or see the full Skin in Evolution™ System.
